Stacey’s Home Health Inc. provides medically necessary, interdisciplinary home health services under the direction of licensed professionals. Our goal is to deliver safe, effective, and coordinated care in the patient’s place of residence in accordance with physician orders and regulatory standards.

Skilled Nursing (SN)

 

Skilled Nursing services include assessment, disease and medication management, wound care, patient and caregiver instruction, and ongoing monitoring of the patient’s clinical condition. All services are provided by licensed nursing staff under physician direction and in compliance with Title 22 and Medicare Conditions of Participation.

Physical Therapy (PT)

 

Physical Therapy services include evaluation and treatment to improve mobility, strength, balance, gait, and functional safety within the home. Therapists provide exercise programs, transfer training, and fall-prevention strategies as ordered by the physician.

Occupational Therapy (OT)

 

Occupational Therapy services include assessment and intervention to increase the patient’s ability to perform activities of daily living. Therapists provide functional training, energy-conservation techniques, adaptive equipment recommendations, and home-safety education as ordered by the physician.
